Please submit your completed Vendor Application by mail and use the link below to pay the $100.00 deposit (minimum) required.  Don't forget to include the Temporary Food Service Permit Application (required for the Health Department) and the Food Service Permit fee of $75.00 (make payable to Columbus County Health Department).
